Alaska Rock Gym Liability Waiver

Every adult, 18 years and older, participating in any activity taking place at the Alaska Rock Gym must properly complete and sign an Alaska Rock Gym Adult Liability Waiver, prior to participation – NO EXCEPTIONS.

All children (individuals under the age of 18) must present his/her parent or court appointed legal guardian, prior to participation in any activity – NO EXCEPTIONS.

A hand written or personal letter type permission slip is not accepted as a substitution for the minor liability waiver. Only Alaska Rock Gym waiver forms are accepted – NO EXCEPTIONS.

Every minor participating at the Alaska Rock Gym must present a correctly completed minor liability waiver that has been signed by a parent or court appointed legal guardian. Mis-conceptions on who constitutes a legal guardian are common – unless appointed by a court of law, grandparents, aunts and uncles, caregivers and older siblings are not legal guardians. Minor liability waivers must be signed by a parent or legal guardian – NO EXCEPTIONS.

Liability waivers may be:

  1. Completed digitally on line from a link at this web-site and submitted directly to the Alaska Rock Gym. 
  2. The blank form may be downloaded from this web-site, executed and then emailed, faxed or hand carried to the gym. 
  3. Or the waiver may be executed in a paper format and presented at the gym prior to participation. You will find buttons to accommodate all methods below.

Please read this instruction in its entirety before reading and signing the Alaska Rock Gym liability waiver. Waivers improperly or incompletely executed are not accepted! You are signing a liability waiver indemnifying the Alaska Rock Gym. Read the waiver completely, fill out the signature and information block on the second page, completely. Every blank must be filled in, and your printed name must be legible! Thank you for your attention to these details.

Minor Waiver Instructions:

Please read this instruction in its entirety before reading and signing the Alaska Rock Gym minor liability waiver. Waivers improperly or incompletely executed are not accepted, and your child may not be permitted to climb! You are signing, as a parent or court-appointed legal guardian, a waiver indemnifying the Alaska Rock Gym on behalf of your child. Read the waiver completely. DISCUSS with your child the significance of this waiver, the RISKS associated with climbing, and the personal responsibility expected of him or her while climbing in the gym. Now take a look at the signature blocks on the second page. There are two distinct signature blocks, to be filled out as follows:

  1. Participant Signature Block – This is for your CHILD! If your child is eight years or older, he signs his name cursively in the blank next to “Participant Signature.” The adult may fill in the remainder of this block. The child’s printed name should be COMPLETE & LEGIBLE. Fill in address, telephone number and birth date. Don’t forget the current date.
  2. Parent or Guardian Signature – This block is where the adult acknowledges his or her LEGAL authority to sign the waiver on behalf of the child named above. This is also where the adult actually indemnifies and holds harmless the Alaska Rock Gym. Printed names should be COMPLETE & LEGIBLE. If address information is the same as above, you may write “SAME.” Don’t forget the current date. Thank you for your attention to these details!
