ALASKA ROCK GYM LIABILITY WAIVER

Every adult, 18 years and older, participating in, or spectating any activity taking place at the Alaska Rock Gym, must properly complete and sign an Alaska Rock Gym Liability Waiver prior to participation.

All children (individuals under the age of 18) must have an Alaska Rock Gym signed by his/her parent or a court-appointed legal guardian, prior to participation in any activity.

A hand written or personal letter type permission slip is not accepted as a substitution for the minor liability waiver. Only Alaska Rock Gym waivers are accepted.

Every minor participating at the Alaska Rock Gym must present a correctly completed minor liability waiver that has been signed by a parent or court appointed legal guardian. Misconceptions on who constitutes a legal guardian are common – unless appointed by a court of law, grandparents, aunts and uncles, babysitters, nannies, and older siblings are not legal guardians.

LIABILITY WAIVERS MAY BE:

1. Completed digitally from a link on this website. It will be automatically submitted to the Alaska Rock Gym.

2. The blank form may be downloaded from this website, executed and then emailed, faxed or hand carried to the gym.

3. The waiver may be executed in a paper format and presented at the gym prior to participation.

1. Who must sign a liability waiver?

Every adult, 18 years and older, participating in any activity taking place at the Alaska Rock Gym must properly complete and sign an Alaska Rock Gym Adult Liability Waiver, prior to participation – NO EXCEPTIONS. All children (individuals under the age of 18) must present a waiver signed by his/her parent or court appointed legal guardian, prior to participation in any activity.

2. I just showed up at the gym with my kids and all their friends. Can’t I just sign minor liability waivers for everyone?

No. Every minor participating at the Alaska Rock Gym must present a correctly completed liability waiver that has been signed by a parent or court appointed legal guardian. Contact the parent or court-appointed legal guardian and have them go to the website and sign one for the minor digitally.
